Instructions to help you register for the portal:
- Click on the blue ‘here’ or link below to access the portal login.
- To register for the portal, click the ‘click here to register’ link below the blue ‘login’ bar (located in the middle of the screen).
- You will need your Student ID number (or staff ID number) and a email address (must be yahoo or gmail email address) in order to complete the registration form.
- Once you submit the information you will receive an email that will contain a link to create a password for your health portal.
- Click the link and create your password.
- Log in to the health portal using your Student ID and newly created password.
- Click the ‘My Forms’ tab located at the top menu bar. All patient forms must be completed before your appointment. If you DO NOT complete the necessary forms, your appointment will be rescheduled.
